2014 NEC - Kitchen AFCI/GFCI questions electricians

I'm struggling to interpret the 2014 NEC as it applies to my kitchen. I currently have:

2x 20A circuits for counter receptacles. (First receptacle in line is a GFCI receptacle on both circuits.) 1x 20A circuit for microwave, which is mounted above the range (it's cord&plug connected to a NEMA 5-20 simplex receptacle in the cabinet over the microwave. This is the only receptacle on the circuit.) 1x 20A circuit for refrigerator (cord & plug connected simplex NEMA 5-20 receptacle behind the refrigerator - not readily accessible. This is the only receptacle on the circuit). 1x 15A circuit - hardwired to disposal. No cord and plug. 1x 20A circuit - hardwired to dishwasher. No cord and plug.

What needs an AFCI breaker, GFCI breaker, or combination AFCI/GFCI breaker? Can any of these circuits be compliant on a standard breaker?
