So many questions so little time! breastfeeding

Ugh I don't even know where to start. My brain is so frazzled.

Issue one: LO is 5 weeks and all of a sudden refusing a bottle. We have been doing a mix of breast and bottle since day two and it has been problem free since finding a bottle she liked (Lansinoh mOmma with the natural wave nipple). Now all of a sudden she is outright refusing to eat from a bottle. We've tried all the bottles you can think of but Lansinoh is hands down her favorite she is just not having any of it at this point in time. I start back to work this week(!!) I originally planned to take 8 weeks but finances (yay -_-) so I'm going back after my 6 weeks of paid leave are up which means I have even less time than I though to work out this situation. I have tried all the recommended things but it's just not working. I left for two hours the other day and she screamed the entire time, her poor dad was going crazy. I work 12 hour shifts, I can't go home to nurse her and her dad will never survive 12 hours of screaming. What do we do?

Also returning to work, what all do I need to take along? We have a pumping room with a Medela Symphony and I have a refrigerator to store milk. I have Medela bottles and tubing but will that work with the symphony?

Last thing: Am I having a supply issue? Last week I was able to pump 8oz with my manual pump, it was in place of a feeding and around 12ish in the afternoon. This morning I pumped using the same pump in place of a feeding at 6:30 am and only got 4 oz. I think baby is getting milk each time she eats, I see the milk in her mouth so that means I have enough right? My breasts never seem super full like they were previously, is this just my supply regulating? Or is my supply dropping?

Sorry it is so long, I'm just worried about so many things here.
